What's the endgame? Onos Health's software scans unstructured therapy notes, intake assessments, and utilization histories to check whether care matches clinical guidelines. Rather than replacing clinicians, it automates chart reviews to help payer medical directors spot high-performing providers and flag care gaps early. The new capital will expand deployment across commercial insurers, Medicare Advantage plans, and Medicaid programs.
Why now? Mental health and substance use care is a persistent blind spot for insurers. Conditions affect nearly one in four US adults and drive more than $140 billion in direct spending each year, yet insurers largely rely on billing codes that reveal little about recovery.
What's the problem it solves? More than 70% of meaningful clinical signals — treatment plans, symptom progression, therapy notes — sit locked inside unstructured narratives that standard claims engines cannot read. The company says its deployments have delivered a 75% increase in clinical review efficiency, a 35% lift in adherence to clinical standards, and a spend reduction of more than 6% within 12 months. "Behavioral health care quality is difficult to understand and manage because it is recorded in unstructured clinical documentation which legacy solutions cannot process," said Akshay Agarwal, co-founder and chief executive officer of Onos Health.
The signal: At $17 million, the round sits in roughly the 73rd percentile for deal size, signalling investor appetite for AI tools that convert messy clinical text into measurable payer decisions. The presence of CVS Health Ventures points to demand from insurers themselves for visibility into a category they have long struggled to manage.
