Aligned Marketplace raises $20M to link employers with independent primary care

What's the deal? Aligned Marketplace, a startup connecting self-insured employers to independent primary care clinics, has raised $20 million in a Series A led by Venrock, Endpoints News reported exclusively. The New York company runs a marketplace of more than 3,000 in-person clinics across all 50 states.

What's the endgame? Aligned curates independent advanced and direct primary care doctors who work outside hospital systems and traditional insurance networks. It sits atop an employer's existing carrier or third-party administrator, so there is no platform switch. The new funding will extend its reach to more employers and administrators, and expand the marketplace into value-based specialty care.

Why now? The company says it has logged 18 months of growth and cost savings with large Fortune 500 employers. In a programme launched 1 January 2025, engaged members at one Fortune 500 employer cost 12% less than a risk-matched benchmark built by a third-party actuarial firm — a saving of $96 per member per month.

By the numbers: Of engaged members, 70% were flagged as high risk and shifted care towards their Aligned doctor, averaging 4.7 visits with that doctor and 1.6 fewer elsewhere. Mammogram rates roughly doubled and colonoscopy rates nearly tripled after a first visit. Some 94% said they could get care when needed, rating the experience 9.2 out of 10.

What they're saying: "Better primary care lowers healthcare costs by making people healthier," said Bob Kocher, partner at Venrock. Founder and chief executive officer Patrick Nelli added: "We've proven that a different model works — one where keeping people healthy actually saves money."

The signal: More than 80% of physicians now work for hospital systems or large corporate entities that profit when care is delivered inside their facilities. Aligned's bet is that independent primary care, tied to engagement and outcomes rather than volume, can lower the bill for the employers who pay it.
