At Home Harmony raises $5M to launch its own Medicare ACO

What's the deal? At Home HarmonyDealroom has a profile for this one. Try Dealroom →, a Henrico, Virginia-based startup offering in-home health services for seniors, has closed a $5 million early-stage round. The money will fund a new Medicare program and expand the company's clinical team.

Who's backing it? The round drew three healthcare-focused institutional funds — Chicago-based Ziegler Link-age Funds, Tennessee-based Claritas CapitalDealroom has a profile for this one. Try Dealroom →, and North Carolina-based Pisgah FundDealroom has a profile for this one. Try Dealroom → — alongside individual investors and founder Will Saunders. "These are folks who are very knowledgeable about the space, and I think that kind of speaks for itself in terms of them backing us," Saunders said.

What does it do? Founded in 2022, At Home Harmony provides primary care, remote monitoring, personal care, and pain management to elderly patients at home. It has seen more than 6,000 patients in central Virginia and employs over 200 full-time staff, including three physicians, 35 nurses, and five dementia specialists.

Why now? The Centers for Medicare and Medicaid Services named At Home Harmony one of about 80 US organisations in a 10-year voluntary Accountable Care Organization (ACO) initiative for fee-for-service beneficiaries. Coverage begins January 1.

What's the endgame? The ACO lets the company offer services such as nutrition and transportation under waived regulations, with patients qualifying based on health conditions. "The capital is meant to kind of launch our ACO, and at the same time, continue to build out our clinical team," Saunders said.

What could go wrong? CMS requires participants to post reserves and prove they have the financial capital to operate. Saunders said the company has already invested over $20 million in its care model.

The signal: Home-based senior care is drawing institutional interest as providers like HCADealroom has a profile for this one. Try Dealroom → and Bon SecoursDealroom has a profile for this one. Try Dealroom → increasingly refer patients to the model. For specialist healthcare startups, aligning with Medicare programs offers a route to scale — and the capital backing to prove it.
