AriBio's lead product candidate is AR1001 (mirodenafil), an oral phosphodiesterase-5 (PDE5) inhibitor being investigated for the treatment of early-stage Alzheimer's disease. Originally developed for erectile dysfunction, its potential for neurodegenerative diseases was identified through AriBio's research. AR1001 is designed to be a disease-modifying therapy with multiple mechanisms of action. Pre-clinical studies indicate it inhibits neuron apoptosis (cell death), promotes neurogenesis (creation of new neurons), restores synaptic plasticity (the ability of synapses to strengthen or weaken over time), and stimulates autophagy to clear toxic proteins like beta-amyloid and phosphorylated tau. The drug has demonstrated good blood-brain barrier penetration and is reported to be 10 times more potent in its PDE5 inhibition than sildenafil.

AR1001 is currently in a global Phase 3 clinical trial named POLARIS-AD (NCT05531526). This large-scale, double-blind, randomized, placebo-controlled study is evaluating the efficacy and safety of a daily oral dose of AR1001 in approximately 1,150 participants with early Alzheimer's disease across 200 sites globally, including the U.S., U.K., Europe, and China. The trial's primary endpoint is the Clinical Dementia Rating Scale–Sum of Boxes (CDR-SB), with topline results anticipated in the first half of 2026.
