Candid Health: rebuilding medical billing as an AI-first stack

Key points

Key takeaways from YC Founder Fireside interview with Candid Health co-founder and CEO Nick Perry (October 2026):

What Candid does. Candid Health automates revenue cycle management for healthcare providers, rebuilding the medical billing stack from the ground up to be automation- and AI-first, so claims get submitted correctly the first time.

Why billing is hard. A single provider may deal with up to a thousand insurers, each with different rules, so the same service produces different claims per payer; a misplaced comma can push a claim out of network and leave the patient with larger out-of-pocket costs.

Market size. Medical billing is a $200-300 billion market in the US, roughly split between inpatient health systems dominated by Epic, Cerner and Meditech, and a fragmented outpatient side with hundreds of specialist vendors.

Go-to-market. The company rode the digital health wave as its wedge: telemedicine-era startups wanted a "Stripe for medical billing", giving Candid a beachhead from which to expand into the wider ambulatory market as features matured.

Manual first. Perry was Candid's biller for the first 18 months, reviewing every claim by hand and building internal tooling, which he credits with teaching the product the long tail of edge cases that make RCM genuinely hard.

AI and agents. The goal is to prevent denials before they happen — a rules engine plus AI agents trained on payer-specific submission quirks, replacing the phone calls and manual detective work that dominate denial management today.

Traction. Candid now serves more than 200 healthcare organisations and processes around $7 billion in claims a year, and raised a $120 million Series D in 2026 to expand into the broader healthcare market.

Background. Perry met co-founder Doug Proctor at Palantir, a data-integration background he says maps directly onto billing; "hair on fire" pain means providers will try almost anything that helps.
